RESOLVED!
I am having problems with the ZAP on a pots line plugged into a Digium 400 4 port fxs card. We have 2 cards, 1 4 port fxo and 1 4 port fxs. The pots line is plugged into the second port on the fxs card making it channel 6. Here is the log file:

The DID, trunk and inbound route are all set up correctly (as far as I can tell) If you need this I can supply it also.
