NO JOY Logging into GUI

I'm trying to log into the GUI where you are presented with the options for either "Admin" or "User":

... but when I go to the server IP (192.168.x.x), I end up being prompted for the userID/PW - upon entering "maint" and the PW, I end up here:

Ideas? I have accessed the primary interface before ... and this hasn't been an issue until now that I have to send a fax and can't get to fax interface. :\

Any help is appreciated.

I get a log in prompt that basically looks like this (except with a different IP and it says "The site says:"Restricted Admin Area"):

127.X is the loop back - what happens when you enter the admin info? I'm wondering, can you even telnet in to the box?

The IP is the PBX IP (192.168.x.x); I am unable to telnet into the box. :\

The "fax interface" is accessed by clicking on AvantFax from the default web page.

Ward that's my problem ... I can't get to the default page. I put in the server IP and it immediately prompts me for a userID/PW from a pop-up window, not the web interface itself. I thought maybe it was a browser issue, but I've tried IE, Firefox and Chrome. :(

Is this a home or business installation?
Can you go to the actual PC running and login?
Did anything change in your network (router,etc)?
What is your ip address on the machine you are using?

I can login locally to the server and I am able to access the admin configuration GUI, it's the initial generic pbxinaflash user/admin GUI I am unable to access (so the webserver is obviously working). The system is an existing system that has been in flawless operation for over a year. The pbxinaflash GUI was accessible in the past (it hasn't been used in 5-6 months). There have been no network topography changes. Any changes that have occurred will have been with updates to the server hosting pbxinaflash. The IP is in the 192.168.x.x range and is preserved below the range for DHCP.

What is the exact server URL you seen after you enter your server IP? I may be able to pull the page entering the .php address for the default page.

After entering my IP I see http://192.168.1.123/ - that's the typical USER menu with Avant fax and such.

What happens when you enter: http://192.168.1.123/index.html or 192.168.1.123/index-freepbx.html (get a basic menu with ARI and PBX Admin).

192.168.1.123/index_custom.php should show you the regular menu you are seeking.

I have a similar issue.

PIAF 3 with IncredibleFax11. If I go to http://pbx/avantfax, I get a login popup that only accepts the "maint" user.

If I go to http://pbx, I get the generic user screen, but it only has ARI and MeetMe buttons. I can I get my users to get to the AvantFax login?
