Is this normal call flow? I just question why a 401 is presented if the extension is registered:

Yes, that is normal. Registration does not "log you in" for placing calls*. Your phone needs to provide credentials for both registration and INVITE requests. This is the same reason why, when you define a SIP trunk that uses registration, you have to put the password both in the Peer Details box as well as in the Registration String box. Peer Details section is used for placing outbound calls, and the Registration String section is used to register for inbound calls.

[*] except in the case of Google Voice "GVSIP" which uses SIP Outbound. Once you register on GVSIP the socket is kept open and you can send INVITE requests down that path without authenticating again.
