QUESTION How to do local Caller-ID lookup on Incredible PBX?

Looking for a link to the current best practice to do local lookup for Caller-ID using a database.

I tried defining a CIDLookup source as MySQL and linked it to a database using the instructions here:

but it looks like MySQL is not supported anymore. We're running the packaged release from a while back Incredible PBX 13.0.192.19) if the version matters

Have you considered using CID Superfecta? I believe it looks at the Astridex database for entries and posts them first. You can also link in openCNAM which is very inexpensive, and it will save those numbers it has looked up.
