GV, Obihai 202 and Incredible PBX 13-13

I had been running my setup for a long time and decided to make the change and try to learn the Obi 202 ATA after having a Cisco SPA 112. I followed Ward's well written directions- http://nerdvittles.com/?p=25846

I am having trouble getting it to work. I don't get any indication of incoming calls, outbound calls work as expected.

Here are my Obi settings-

Question: On the port number for the trunk - my understanding is that it should match the value in X_UserAgentPort parameter for SP1 service. (https://cboh.org/voip/obi/OBi_As_ITSP.html)
Ward's article suggests trying ports 5060.....5063. I actually put Ward's article and Rob Stampfli's article into a spreadsheet just so I could see the differences and similarities of the two configurations side by side. Both people say they got it to work. I'm sure it does but I'm not having much luck with either method. I did that en an effort to understand the OBi because it is nothing like the old SPA 112 I had (at least to me it's not).
It sort of seems like two different methods to arrive at the same result, an OBi GV to PBX bridge.

Does anyone see any obvious mistakes with my setup? Many thanks in advance for your assistance!
