Who do people use for toll free? Cheapest I have found so far is 2.5 c Canada and 1.5 c US (in USD, subject to regular USF recovery taxes and other stupidity).

No cost TF termination providers I have used via my trunks:

Sip Denetron - SIP/[email protected]
Toll Free Gateway - requires registration - SIP/[email protected]
TF CallWithUs - SIP/[email protected]

Also, VoIP.MS with tf routing checked.

Sorry, not enough coffee...

At any rate, I have a TF number via VoIP.ms. Their termination rate is .0240 per minute and a monthly fee of only .99 I tend to look at the monthly fee first, then the minute charge. Ours is for family and it works just fine.

I'm confused about what toll free origination means. Is this dialing out from a toll free number? If so, why?
flowroute lets you set the outgoing caller id, so you could dial out on a 'regular' line and set it for your 800 # so a callback could come in through that route.

Am I missing something?

Origination is the terminology used for inbound calls, ie DID's.
