When we receive or send an external call, we do not receive any sound but the other end can listen to us.
We have to terminate the call and recall again to complete the conversation. This problem is internitent.
We do not have the problem when we call an extension Inside the building.
Phones are Grandstream GXP 2160
Server is running Asterix 11.10.0
It is behind a router Cisco WRVS4400N

You may want to check rtp.conf for rtpstart and rtpend, by default 10000-20000. You can use your router to forward this UDP port range to your PBX. Many routers will not require this, but perhaps yours does.

10000 ports is a great deal, far more than you need unless you are handling thousands of simultaneous calls. You need two RTP ports for each call. If for example you expect to have a maximum of 10 simultaneous calls, you could set rtpend to 10100 which should give you plenty of room for growth, and only forward 10000-10100 instead of 10000-20000. (Restart Asterisk after making this change.)

Does that solve the problem? If not, report whether audio travels directly to your phone or is proxied by the PBX.

There is LOTS of information on this that all relates to routers, firewalls, keep alive settings, NAT, turning off SIP ALG and port forwarding. You have a lot of reading to do. Routers and firewalls are only simple when you are using them for web surfing. As soon as you add a server or VPN the complexity goes up from there.
