User extensions set up as PJSIP (they don't show in the Statistics panel). The asterisk -vvv -r dump shows a ton of this:

Your first problem, is that CAC hostnames, do not resolve on the internet. Adding it to the hosts file is the way around this.

Or, you can add a new name and address to a DNS domain that you control. The former solution doesn't help with external hosts (such as, extension phones) finding your server. That may be a plus or minus, depending. The latter may also allow you to add server records to the DNS which may be a plus for you.
