BUG Ubuntu 14.04 Upgrade Bug

Just an FYI that Ubuntu has introduced a problem with their latest MySQL upgrade. Not sure what it is just yet, but be forewarned that running apt-get upgrade before installing Incredible PBX will cause the install to fail because attempts to stop and restart MySQL fail which causes all sorts of collateral damage. Tutorials for Incredible PBX 11 and Incredible PBX 13 have been adjusted accordingly. Here's the safe installation procedure for Incredible PBX 13. Be sure to uncomment the create-swapfile-DO line if you're using the Digital Ocean platform. Special thanks to @kyle95wm for the heads up.
