    It was too early to declare victory. Some calls are connecting, others he hears no ringback, a half ring, or one ring and then nothing.

    On a whim I changed his DTMFMODE on the pbx from sip info dtmf relay to rfc2833. He made a series of successful test calls. Too early to declare victory but if this is it, someone smarter than me will have to explain why it worked before but not behind this extra router in bridge mode.
