NO JOY Unable to add second extension on Polycom VXX500

Map an extension to a Polycom VXX 500 and it registered successfully. Tried to add a second extension, but not able to register. Please help. Thank you.

Asterisk version 11.10.0
FreePBX version 12.0.76
