1. Yes it hosted on the same LAN
2. I'm using firmware 8-5-4TH1.6 (app load id)

When I was running sip firmware, I would need to have NAT set to ON on the phone and OFF in freepbx for them to reliably connect...

The issue with GV is that it will work great for a month, then stop working, or fail in some different way. Asterisk will pass the call to GV, and GV will just ring forever, or drop it, or hang, all while reporting to asterisk that all is well. To really make this work reliably, you have to use a real sip provider.
All of us here have fought with GV for months before giving up and moving on, so we don't want to fight your battle with GV.

OK, so I've purchased an unlimited, 1-year account with Vestalink (previously Obivoice) and have moved their trunk to the top of the priority chain for all domestic calls. I'm still a little confused about the reported issues from extension to extension, but I suppose all I can do for now is keep an eye on it going forward.
