NO JOY inbound root going to voice mail instead of ringing phone?

Hello am running in to a strange issue with inbound roots when ever I set my inbound to extention it automatic goes straight to voice mail instead of ringing the phone, the phone is registering to the extention fine. currently I am testing set up with localphone inbound service, tried turning off permently iptables and fail2ban and still same results any idea what is going on? my system is incredible pbx on sent os 6.5 32 bit digital ocean is the service I am running this on. also I just tried switching the inbound root from extention to disa and that works perfectly fine I was able to dial in to the system and perform a echo test. so it appears that something is wacked with extentions, any one know what is going on? have never ran in to this before where the extentions don't work but every thing else does

You mean route.

Is the extension set to Do Not Disturb? You can check what's happening looking at the /var/log/full log and finding the call and seeing why it goes to voicemail.

where do I see if the extention is set to do not disturb? didn't even know it could do that. also below is my log file
