NO JOY Calls go silent during voicemail greeting outbound calls

I appreciate in advance any help given in resolving this situation. I have an issue where on some calls to external numbers during the voicemail greeting the call just goes silent. The problem is somewhat random but some numbers do it more than others. It seems to happen more on phone systems than cell phones. I have done searches and have not found similar issues for outgoing calls.

I am running PIAF 2.0.6.5 with Asterisk 11.7.0 and use a trunk with Vitelity.

I have attached the asterisk log from the call and below is what Vitelity sent back to me.

We did not receive the ACK after the 200 OK, causing the call to tear down and your device just sends the cancel repeatedly. There may be something preventing your device from receiving all the signals. "
