I'd check in Freepbx under Extensions -> 503 and see what it shows in the "Dial" field. Does it have SIP/505 or SIP/503?

You nailed it! The lesson is check and re-check! this PBX has been running for more than a year but no-one was using that extension so nobody noticed this behavior!
