NO JOY unidentified or withheld and therefore cannot be called back

I'm trying to figure out something my system has just started doing. It is FreePBX with everything up to date. (Version number tomorrow when I can check in person.) I have two incoming routes I expect people to call and a third which is my Callcentric account number.

The first, is worked fine and continues to work fine.

The second, worked yesterday. Now, incoming callers are told "unidentified or withheld and therefore cannot be called back". I do not know what made it start giving this error message instead of connecting. I've been assured that no changes have been made.

This has something to do with Blacklist (12.0.1).

In my log file I get this:

Uninstalling Blacklist removes the problem, but then nobody can blacklist people. Reinstalling it brings the problem back.

I can post a longer log if it would help.

Partial answer -- a Cisco phone got stuck on "Forward to *32". I'm not sure how that condition got created or how to prevent it in the future. With Blacklist uninstalled, that phone just wouldn't ring. With it installed, I'm guessing everything got passed immediately to the blacklist.
