QUESTION How to install PIAF on OpenVZ VPS without template?

I have a PIAF installation running on an OpenVZ VPS at RamNode. It was installed from a template that they supplied and it runs fine.

I'd like to build a "warm spare" on another OpenVZ VPS at NexHost. They do not offer any PBX templates, nor do they permit installation of one's own template. Available templates include both 32- and 64-bit CentOS 6.

Please excuse the stupid newbie question, but I don't know how to proceed. I tried following various tutorials, but they all tried to do prohibited or irrelevant things such as modifying the kernel, configuring grub, etc. Searching this and other forums for OpenVZ, I've found only unresolved threads and pre-built templates. More general VM searches yield lots of info for VirtualBox, VMWare, ESXi, KVM, ..., but little about OpenVZ, even though that's the most common VPS.

Also, the cheap "DollarVZ" VPS does not have a dedicated IPV4 address -- you are behind a NAT with a small range of ports forwarded to your VPS. Will this pose any problems? (I'd set bindport, rtpstart and rtpend accordingly, wouldn't want to listen on 5060 anyhow, and would use VPN or SSH SOCKS proxy for management.)
