Also update, we had a box go down today and I looked at the CDR logs and found this oddly enough. Is someone trying to brute force my box? what could cause this? Anyone with any ideas?

Check your SIP settings and make sure that Allow SIP Guests and Allow Anonymous Inbound SIP Calls is set to NO, that should take care of what you are seeing in your logs.

This. Definitely looks like you have SIP open to the world. Do you have any remote extensions or reason for it?

As for the pfsense, I don't use it but I recommend checking out this article and see if it helps.

https://doc.pfsense.org/index.php/Asterisk_VoIP

I use pfSense all day every day for two different PIAF boxes. Don't recall ever making any special changes to make it work. For one of the two, I do have 1:1 NAT enabled + Ward's whitelist with all traffic passed through pfSense.
May try for the heck of it making a firewall rule for 5060, 10000:20000 and allowing it to/from the PBX.
